Form field not showing

one field is not show up in the form on my web site. It is called Web Site I want to capture the clients web address.

The field "Website" is not showing because you are using a condition that will show or hide form fields.

You need to select "Others, please specify below." in order for the website field to show up.

You can just remove the condition so that the website field will always shows up.
