why is my form disableed

Your forms have been disabled because they were used to collect Credit Card information, which is against our terms of service.

Please remove the offending fields from all your forms - Credit Card Number, Expiration Date, Security Code, Card Holder Name. The only way that you are allowed to collect such sensitive information is if you use one of our Payment Tools to collect payment information, and to use secure SSL submissions for collecting these payments.

After removing such fields, our Phishing detector tool will automatically re-scan your forms, and re-enable your account if the forms are no longer violating our terms. I hope this helps.
