In our application process there are two forms with similar information.
I would like to have name and address automatically transfer from this form: http://form.jotform.us/form/51805036938156
To this form when the submit button is selected: http://form.jotform.us/form/51946811215151
Is this possible?

If you want to automatically pass the data from the first form to the second form after submission, you can do so by following the steps from this thread:

If you want to move the data from the first form to the second form, you can do it using our import app:
http://www.jotform.com/help/308-Import-App-Easily-import-your-data-in-Excel-or-CSV-to-JotForm
But before using the app, make sure that you have already exported the submission from the first form into a CSV file (http://www.jotform.com/help/103-How-to-create-a-CSV-Report ).

http://form.jotform.us/form/52076288469165?lastName={lastName128}&firstName163={firstName129}&middle={middle}&emailrequired95={emailrequired95}
I am not exactly sure what to do with it now.

This is the correct URL:

Last Name field name: name308
First Name field name: firstName
Middle field name: middle
Email field name: emailAddress
Second form: http://form.jotform.us/form/51946811215151
Last Name field name: {lastName163}
First Name field name: {firstName129}
Middle field name: {middle}
Email field name: {emailrequired95}
Again, add the above prepopulated URL as the Thank You URL of the first form.
