We are getting applications with missing data for fields and data in fields where it doesn't belong? Fields don't correspond with the correct data/info?
We are having major issues with our form that was uploaded by JotForm from a PDF that we submitted. Can someone review our form to help with this situation and make suggestions on what needs to be done?

I did check your form and found that you have edited your notification email alert. Please be noted that if you edit your email alert and then make changes to your form, the changes will NOT be reflected in the email alert.
To solve this problem, I would suggest you to delete your existing email alert and add a new one. The following guides should help you:

Unfortunately, imported PDF forms cannot be edited. This includes the email notifications. You would need to recreate in JotForm in order to be able to edit it. Subscribing will not change this. We actually do not limit any features to subscribers only.

tfiscusReplied on July 27, 2015 at 3:44 PMCan we delete the current form and resubmit the application to be re-imported. Would it be better to embed the form on the website than to save a copy of the imported form to access on our website? thanks

If you are importing a PDF form, it will not be editable. If you have the web version of the form, importing that would create a an editable version of the form. The PDF copy also cannot be embedded. If the imported form has [PDF] at the beginning of the title, it is a PDF form and only works properly in PDF form.
