The problem is that you have a form with the word "codigo" (code) which is part of the keywords the phishing detector handles. Please replace it with something different, such "activation number" and for the captcha just leave it as "captcha" or something different. Otherwise, anything related with access, code or password will be marked immediately as suspicious of phishing activity.
