Submission not showing in the spreadsheet or PDF

Also when the form is completed, the responses to these specific questions do not show in the submissions spreadsheet, nor the pdf, they only show in the email. Please can you help.

I cannot replicate this problem to your form. The data is there and I see no issues about it. Here's a screenshot in the PDF:

I think you have sorted this out already. I hope so. But if not, please let us know so we can investigate further.

I would also appreciated if you can share us the files, the PDF and the spreadsheet so we can check.
