    3) When I went to the link you provided - I read a couple of comments.  One said he needed to be able to print the submitted form in the same format as is seen in the Form Builder.  The answer said it can be done, but didn't really explain how.  When I click attach PDF to the Notification, it sends a PDF with all the information but does not look anything like my form.  Is there a way to create a PDF that looks like MY form?  If so, how.  If so, can it be attached to the notification email.

    Hi,

Unfortunately, it is not possible to edit the style of the PDF submission so we can't achieve this with PDF.

However, form notifications are highly customizable. You can format your form notification to anything you want, you can also mimic your form style but you have to work on this manually.

With regards to copying your form layout, simple paste your form source code on the source editor so it will import the skeleton layout of your form. Unfortunately, just like regular emails, it does not execute scripts and run style-sheets so you will definitely not get the exact layout of your form when you paste your form source code but you can continue working on the styles by manually editing them using the formatting toolbars on the top.

    Hope this information helps!
