How can I get a smooth signature box to appear on the for after I have saved it to a .PDF??

Are you trying to create a PDF of the form submission:

If that is the case, the smooth signature does appear in the PDF.

However, if you are trying to created a PDF form, widgets, such as the smooth signature, will not work:

There are limitations to what the PDF version of forms can do and widgets are unfortunately something they cannot handle.

I am trying to create a PDF and be able to see a box with a blank line so that once the person print the document, they can PHYSICALLY sign it.

Unfortunately the signature widgets do not work in the PDF of forms.  You can, however, add a print button to the web version:

You could then print the blank form with the signature line included.
