To confirm, you would like to add a file upload function or feature in your form, is that correct?
You can use the default File Upload field found under the "Form Tools" section
When using the default File Upload field, it will directly be uploaded to our servers.
You can also check this guide: http://www.jotform.com/help/332-How-to-create-upload-forms
Another information that I can share, you can use the file sharing storage and services like Dropbox, Google Drive and Box.net to your form that has a File Upload field. Here's the app page for them: http://apps.jotform.com/category/file_sharing_and_storage
These guides on how to set them up might help:
