Hello,
I had my form attached to google spreadsheets, it recently stopped recording all of the submitted field data and just records:
First Name
Last Name
Email
Ip
Submission Id
I would like for it to record all of the submitted form selections.

Our apologies for the issue.
I would like to know if all of the fields are going to Google Spreadsheet before? And if the integration is already connected, we do not advise doing any modifications to the form fields and the spreadsheet file because that would break the integration.
But since the problem is already there, the best way you can fix this is to re-integrate the form to Google Spreadsheet. The integration will give you a new sheet with all of the existing form submissions.
