None of the information from the top above the signature box is showing up

We would like to have everything from the very top of the page on down to show up on the page. This ensures that everything is legally-binding. The only thing that we are seeing once we fill out the information, we are just getting the signature box information. Thank you.

Is the data information you are receiving in the email, kindly remove and add a new notification email this should update with the form fields.

Once I fill out the electronic information at the bottom and click submit, the information that the applicant has filled out that describes what they signed for is not connected with their signature. When I receive their information, I want it to show everything. Thank you.

To add the mentioned information you will need to manually put it in the email template since its not part of an input form it wont get picked up automatically.
