I tried submitting a ticket earlier, but i couldn't find it and i'm not sure if it went through...
So i'm sorry if this is a repeat.
My "apply now" button form stopped working recently, and i'm not sure why.
Could you please take a look and see what the issue is?

I did check your web page where you have embedded your feedback form and it seems the script code is actually commented. Please check the screenshot below:
I would suggest you to please grab your form's feedback form and use it in your web page. The following guide should help you how to grab your form's feedback form: http://www.jotform.com/help/93-Creating-a-Feedback-Button

I didnt embed a feedback form, I wanted it to specifically be a blue button that lightboxes up.
The code I received was working, and now it suddenly has stopped.

What my colleague, Ashwin, meant was you have embedded the Feedback button on your website that will show the Form once clicked.
I have checked your website: http://boostpros.net/contact and the button were not there already.
Are you saying that when you include the part that's commented out, your page breaks? I just would like to clarify that, because I believe that commented code is needed for it to work.
I'm not sure how it works before without it. But, can you provide some non-public page that we could access with the original code as he got it so that we can inspect it?
I understand that you're using CloudFront and what Ashwin is suggesting is that you use the Feedback Button script straight from JotForm to your website and see if it will work.
