What makes your ad unique field does not send info. I get all the other fields in email but not that part. Can you help? Or is there a better form to use?

What field of your form aren't you receiving in your email notification? The Unique ID widget field? You haven't added it to your form:
If you add it, and it does not show in your email notification, simply re-do it: http://www.jotform.com/help/25-Setting-Up-Email-Notifications
