    Is the information in the form confidential, where will the data be stored, can I attach things before submitting it

    I am a co-ordinator for hand trauma surgery for an area of South West London. Hand trauma that is seen in hospitals, GP practices and urgent care centres all over our region is sent into our hospital for management.

    I would like to design a referral form for doctors outside the hospital to use when referring patients to us.

    Has Jotform been used for this type of application before?

    Is it possible to guarrantee the confidentiality of the data sent on the form?

    Do you store the data or is it all uploaded to our server and stored there?

    If you store it how is it backed up?

    Is it possible to attach xrays to the form before submitting it?

    Is it possible to design the form so that if we can add information and or graphics to it when it arrives at our hospital so that we can keep all information relating to a patient on the same form?

    Thanks for your interest in JotForm. 

    I have passed your query onto JotForm management.

    Although JotForm is most likely capable of providing the functionality you require, I am sure data confidentaility is your prime concern here and I want to be completely sure of the accuracy of any responses offered to your questions.

    Yes, I know there are some healthcare professionals using JotForm.

     

    Is it possible to guarrantee the confidentiality of the data sent on the form?

    We are doing our best to make sure all data is kept safe and we do not share your data with any third parties. 

     

    Do you store the data or is it all uploaded to our server and stored there?

    We have both options. If you use www.jotform.com, your data will be saved in our servers. You can also license JotForm Application and get it installed on your own server and in that case, your data will not leave your servers and it will be completely private.

    We back it up on our own servers. They are all located in United States.

    Yes, you can attach large files to the forms.

    Yes, it is possible edit existing submissions.
