Combining two or more forms

    I tried to follow the instructions in the 7/24/14 "How can I combine two forms together?" forum posting and didn't get the result I was looking for, which was a longer form combining the "Clone of Patient Form - Page 1" with "Clone of Patient History".  I added the source code that I got from the 2nd form into a text field just above the submit field on the 1st form, producing this image.  The ">" is where the text field is located.  I must not be following the instructions properly.

    Use the iframe embed code instead. Script embed code will probably be stripped off due to the security filter imposed on the text tools.

    Please be reminded that you can't combine the data on each of the form. You can only embed forms to another form but that does not mean their data can be combine during form submission. To clarify, when you hit the submit button of the first form, only the first form will be submitted. The same situation will happen on the embedded form, only the embedded form will be submitted when you click its submit button.

    Based on your response, I’m not sure the purpose of combining the forms. We would want the data from both forms submitted on the email notification and pdf.

    It sounds like the only way to combine the forms is to add the text boxes, etc. from the 2nd form manually to the 1st form as if there was only the 1st form. If that’s the case, no problem, I’ll just work on that.

    Were you perhaps looking for the JotForm Field Manager App?

    The app will allow you to combine the fields of 2 forms so that it becomes a mixed of 2 forms.

    When I try to get to the Field Manager App and log in with my normal JotForm ID/PW, the screen does not go anywhere, nor does it give me an error message. The log in screen just freezes. Do I need to set up another account to get the app? If I do, how will it get to my existing forms to combine them?

    Kindly login to you account firs the access the field manager app also clear you browser cache then log back in.

    You do not need to set up a new account

    It was an IE11 issue. I went in via Chrome and it worked fine.

    My issue now is the font size and line spacing in Chrome versus IE. Printing my form out of Chrome creates 8 pages versus 5 in IE. I’ll do some searching on the Forum and submit a separate question,
    if I don’t find an answer.
