fields are not being filled

    Do you mean to say that the email received is having the information filled? I see that the email notification on your Jotform titled "Contact Pastor Tina Ovia" is having different fields than the one having on forms.

    You may remove the existing notification and then add a new notification so that the fields on your Jotform will be automatically added.
