Can you please give us a bit more details in order to be able to see what might have happened to it?

- landoneAnswered on August 04, 2015 at 06:35 PMToday when I was testing my newsletter slated to be sent before 2 PM, I found no active link to our Jotform order form. The form was missing from our website no matter how I approached the site. Our IT guy restored it to working condition and orders are currently arriving as usual. He said I had deleted it when I removed all the orders from last week by deleting submissions as I do each week. I did nothing unusual and can’t find where I was even here yesterday in the history. Can you tell me what happened please? If it happens again, and he is unavailable, are you available to help restore it?

and I can see the embedded JotForm on it.
When I checked the form http://www.jotform.us/form/51201758559156 I found that it belongs to a different account/username than what you have used here.
The form owner is TheShrimpConnection
I also checked the form's submission data and I look like everything is fine. I can see the existing submission data of the form.
Are you also the owner of the account TheShrimpConnection ?
If you are, you can also just login to the account at http://www.jotform.com/login and check the submission data.
