    Hello nickshort,

I did check your form's notification email template and found that the field name of the arrival date/time is not mentioned correctly. Please check the screenshot below:

It seems you edited the email template and then made changes to your form fields.

Please be noted that if you edit your email alert template, it becomes dirty. That means after editing email template, any changes you make in your form will not be reflected automatically in the email alert. You will have to manually make the changes (add removed form fields in email alert).

The best way to solve this problem is to delete the existing email alert of your form and add a new one. A new email alert will always have all the form field with correct field names. The following guide should help you:

    Delete email alert:   https://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder

    Add new email alert:  https://www.jotform.com/help/25-Setting-Up-Email-Notifications

    Hope this helps.

Do get back to us if the issue persists in new email alert as well.

    Thank you!
