I am not sure if I am doing this correctly. I tried a lot of ways to embed this form and it is not displaying. If I copy the link and paste it on my site then it will display.
When customers fill that form out and submit it, the form is not going to the directed emails.
Please help me!

I have checked email address that is set inside of your email notification and that email address was on bounce list. Our system gave following result:
firstname.lastname@example.org is IN the bounce list
Reason(s) : Amazon SES has suppressed sending to this address because it has a recent history of bouncing as an invalid address. For more information about how to remove an address from the suppression list, see the Amazon SES Developer Guide: http://docs.aws.amazon.com/ses/latest/DeveloperGuide/remove-from-suppressionlist.html
I have now removed it from our bounce list and it should be working fine now.
