I have added a response button to my form, but when a form comes in there is no record of the question

I recently added a response button to my form, but when someone sends in the form, there is no record of the button. I must need to add or do another step, can you help?

I have submitted some test data using the form and the data went through okay. Also there is a hidden captcha field in your form this causes it to come up on after the submit button has been clicked, you might consider removing it if you do not require it.
