Do you have custom table (allows you to arrange and repeat questions of
different types in a table format )
    Dropdown Menu grid
    Radio button grid
    text boxes grid
    grouped questions
    email type
    essay type
    date type etc.

There are various widgets that might be useful for you for what you are looking for, but you would need to check each one to see if it is just right for what you need.

Now my recommendation is to build a form and if you find yourself not being able to find the field that would allow you to do something, to just let us know over our forum with the details about the field and we would be happy to find the field that suites the requirements the most, if there is not a perfect candidate already.

Of course, we can also raise threads as feature requests to our developers, so it is easy to have even more options available. :)

Based on the above however I can list out few fields:

    Configurable list

    Infinite List

    User Contributed Radio Buttons

    User Contributed Checkbox

    User Contributed Dropdown

    Field Multiplier

    Add Options

    Matrix Dynamique

There are many more widgets that you can go through and see if any match your desired outcome and we will tell you more about this on your other thread here: http://www.jotform.com/answers/629617

I would just like to add that to add any of the widgets to your jotform, it is very easy, but there is a guide that will help you see just how to for the first time: How to Add a Widget to your Form
