For some reason, I can’t go to the revision history for any of my forms (see the screenshot attached ‘Observations – Revision History Missing’) but I am wondering on what day the form was changed. I remember that I changed all the ones in this format within about a week or less, after there was a request for the change, but the attachment ‘Preschool observations – Excel doc’ has submissions as late as May 20 without the matrix answers. Is there another way to see revision history for these forms?

We have a temporary glitch in the system, this has been fixed, please go ahead and try the revision history: https://www.jotform.com/help/294-How-to-View-Form-Revision-History

Thanks!

You can clear your browser cache, and log in again, it should not affect anything in our system. :)
I have logged into your account to check, and I do see that there is a long revision history on your forms. After clearing your browser cache, please try looking at the form revision history again.
The dates on the left are expandable / collapsible, they hold all changes made on that date.
Please let us know if you need further assistance, and we will be happy to help.
