Can you create a jot form from a an existing pdf document?

    Apologies but we do not support importing PDF form to Jotform. You can however submit a request and attach your PDF so our team will be able to consider creating your form.

    To submit a request, please click on create form button you see on your my forms page then select the last option on the window.
