I have had two submissions. One today and another a few days ago. But there was nothing that came through and our donations did not process (hence no info). How can I figure out what is going on?Page URL:

I checked your form's Revision History and I noticed that the form has no required field just a little while ago.
So this might cause the blank/empty submission in your form. Since your form allows submission even if their is no data entered in the form. I can see now that you have modified this.
Can you try to do test submission and see if you are still receiving empty/blank submission?
