I received this email from someone trying to complete two different forms:
"I am currently in the process for trying to complete and sign the two forms you sent to me, the Service Day and New Student Retreat Liability Forms, but when I hit submit, it erases my signature and proceeds to take me to a screen telling me to go back and sign. The screen that pops up tells me that my signature is required even though I have already signed the form. I was wondering if you could help me pinpoint the problem, and what I need to do to submit these forms."
In the meantime, I unchecked the "required" button for the signature line to see if he can get it submitted that way.
Any suggestions?

I did check your form and I am unable to replicate the issue you are having. I was able to submit your form successfully and It seems to work as expected. Please check the screenshot below:
