i have pasted the embed code to my website but i am not receiving emails from the form.
Seems there is no embedded jotform in your site, but I upon looking your last form edit, I may be referring to this form: http://www.jotform.us/form/52175829732159
I see the sender name is missing in your notification:
Sender Name :Make sure you assign it to a field from your form where the user enters her name (usually the"Name" field -or similar)
On regards of embedding the form, I would recommend you to embed it with the Iframe code: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
Paste the code in a text editor first, you will notice two part of codes, just copy and embed the iframe part:
