I took the source code provided by Jot and added it to the correct location in Xara Web Designer's placeholder.
The form looks good in preview mode and all of the entry fields populate fine. The problem is when I included an attachment, it never arrives in my email inbox. What gives?
I haven't exported the web page out yet to my server, but if and when I do is there perhaps another folder of files that I need to include with the webpage? (ex. css or .js files?) Is this the issue with the attachments not sending?Also, how do I get the submit button's notification message to go to a new page on my Xara Site rather than the default "Thank You" notification section that I had setup with Jot? Something I need to edit in a js file or something?

There's no need to include additional codes. You just need to update your form notifier and make sure to include the upload field name in the area. If you can give us the form URL, I would be happy to fix it for you.
Or, you can simply delete the current notifier: http://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder and re-create it after: http://www.jotform.com/help/25-Setting-Up-Email-Notifications.
