Email Notification options - Missing

Hey Guys!

When creating the sender name, and the reply to fields in my notications on two of my forms - the fields that I want appear blank, and/or are missing from the drop down menus in the setup screen.

I've attached visuals.  I'm trying to create the following:

The Sender name is the "STAFF NAME", and the reply to address is the "STAFF EMAIL ADDRESS"  both fields are towards the tops of both forms.

I checked this on one of the mentioned form http://form.jotformpro.com/form/51543959296974 and I was able to understand the problem..

The thing is, the field names STAFF NAME and STAFF EMAIL ADDRESS doesn't have label name. The labels of this fields have its visibility set to hidden.

So on the Notification Email template they also appears to be hidden.

BUT they were actually there.. you will notice this if you take a closes inspection, you will see on the empty gap that there were horizontal like lines that appears that actually were placeholders for the hidden fields label name.

Please see on the image below for more clarity on what I was describing..

You can still add those hidden field names using only those invisible placeholders, you just have to make a guess which field they actually were. On your form, I think its the 4th and 5th empty labels... that is if you follow the sequence arrangement of those fields on top.

Please try adding them also on your form Notifier and do a test submission just to check if the fields were assigned.
