How I can delete my account?

I want to delete my account. I can not create an account on more than one form, that's why I want to delete my account.

Your account was AutoSuspended as what I have checked also but, I have cleared it already as I can verify that the forms on your account doesn't violate any of our Terms of Use with regards to Phishing Forms.
