Refresh form after submit

Hi, on a feedback / query form, after the user has entered their info such as dates and chosen drop downs etc, how do we get the form to clear after they hit the submit / send button?  I have the setting such that it goes back to the same page, rather than a Thank you page.

Inside of your form you have Enabled Auto fill feature. If you want that form is clear after submission you need to disable Auto Fill feature.
