Form doesn't display correctly on Safari

    The form on the page link listed below doesn't display on my iphone in the safari browser. Particularly, the words next to the icons doesn't appear.  See the attached document for the screenshot.

     

    How do I resolve this issue?

    Can you please try also re-embedding the form to your website using instead its iframe embed code .

    Perhaps its a script conflict that is preventing the media scripts from working correctly on the website. Using the iframe embed code should eliminate this script conflict.

    Please let us know if this did not make any difference.
