Why are my forms not found and disabled?

Hello.  I'm trying to evaluate the system by testing some forms.  It seems all forms I test are not found or disabled, even though I have not disabled them.  Example:  http://form.jotform.us/form/52216388513152

I am just trying to click "Preview" in the form builder.  Can you please help?  Thank you.

It seems that your account has been automatically suspended by our Phishing Detector tool. It may have been done due to a form that you have deleted recently, as I do not see any active forms in your account that would trigger the automatic suspension.

As I can't see anything wrong at this time, I have set your account back to Active. You should be able to use your forms again.
