Displaying forms in apps

Hey there, I would like to create a simple apple and android apps to display my forms and to allow payment and booking of appointment through the app, can you give me a quote to achieve this?

I'm not sure what you mean but you can try to display your form in your apps by downloading your form's source code and embedding it in the app you will make. You can also try ionic framework or any other applications that can make mobile applications with html in it.
