The form must however consist of a graphical analysis of the results, there will be some calculations on data i.t.o allocating a value to the answer and determining the % score. There must also be a graph on this form indicating the answer spread. Can Google Sheets help here - but then only information from a company or consultant must be visible.

Will the collected data originate from one form or it is from many sources.

If it is on one form you can use our report tools to generate a visual report which can then be exported as PDF and emailed
