I requested a form and don't know how to make corrections

Hello

I need to make adjustment on form not sure how to do or if you can do.

I see if gender is equal to male show military.  Either sex can be military.  I need to delete N/A cause I need to know the sex of the person to book cruise.

On emergency contact I need to know the relationship of the person

I need to add dining times as early, late or your time

I need to add prepaid gratuities yes or no

Thank you again for your assistance.  I will greatly appreciate if you can make updates or show me how to .  I did make a few other updates to form.

Hope you are fine today. Please open the form in your account.

1) Remove condition to show MILITARY option for MALE only

Please click on the CONDITION button on the top toolbar.

Now, just click SAVED CONDITION located at the bottom of the window.

A new window will pop up with all the condition created. to remove any condition hover over the condition. On the right you will notices some icons appearing. Clicking on the red icon will remove the condition.

Once finished, click the FINISHED button to save and close window. You will notice that the Military option is no longer hidden.

2) Remove N/A from sex option

Click on the SEX field. You will noticed the following

To remove any option, click on the option and you will again notice some icons appear on the right

Click on the red minus icon to remove option

3) Adding relationship field/ dining times/gratuities

There are different types of fields you can use textbox, radio buttons or dropdown. To add any of this fields, you just need to click and drag the field to any place you like.
