I am trying to embed a form and it is not showing up on my page.

My form should go on https://dpnicoli.squarespace.com/quote-request-form/

I tried both the javascript embed code and the iframe and it is not working.  This is a Squarespace site and I put the embed code in a SS code block.

I have checked your web page and have found your form in it. 

Were you able to find a fix already? Your form, as you can see on the screenshot above, shows on your web page.
