You can have an interactive form by using Conditional Logic to interactively show / hide form fields based on what your users have selected on your form.
As it is not entirely clear from your screenshot, it looks like the elements on the left may be form tabs (Flights, Hotels, Flight+Hotel, Airport Pickup). If that is what you are looking for, we do provide the Form_Tabs widget. (manual: How to Add a Widget to your Form)

I want to create a form with the options to select tabs
and how can i create multiple fields in the same like as well.

As stated by my colleague, you would use our Form Tab Widget to get tabs on your form. You can add multiple fields in the same line by using our shrink option:
