Though you cannot directly import MS word format to JotForm, you can try to convert the word file to HTML and then import the generated file to JotForm by cloning it. If you are willing to use this method, you'll need to publish the generated HTML file first to a web server in order to be able to clone it using its URL.

Also, please note that cloning a form will only import the form fields without any CSS formatting, but it might save you some time in terms of creating each field manually in case the form is too long.
