Hi,
I checked your form and it looks like the notification is properly set up. So, I checked our mail logs for the address in the notification and it looks like your notifications were being marked as spam by your provider:
This resulted in your address being placed in our bounce list, causing your notifications to fail to be sent.
You should change the subject line of the notification for your form from "New Submission: {Form Title}" to the actual title of your form to prevent further filtering.
