Form never sends

    When you go to submit a form we created the button changes to "Please Wait..." and never submits. Also when you hover over the button it says that the form is encrypted. While I looked into that I decided not to use encryption so not sure what is happening here. (please ignore the attached screen cap)

    There is a conflict between your page codes and embed code of the form. Please remove this line

    <script src="http://form.jotform.us/jsform/50466256275156" type="text/javascript"></script>

    And replace it with the iFrame code of the form.
