Base on your screenshot, the Jotform resources are not loaded properly on your browser which is fine on our end. Could it be due to your security software that blocks the resources URLs from Jotform? I would suggest to check it on your security software and whitelist those URLs if there's any. This should resolve the problem.
Another thing to check is your browser cache, try clearing your browser's cache and see if it makes any difference.
May I also suggest to check your internet connection, and perform a DNS cache clearing if necessary. Perhaps this would help.
