Terms and Conditions Widget not submitting the form

Hello. 

I submitted a help question about a week ago. I am still having problems with the form that I built on here. I have narrowed down the problem. http://form.jotform.us/form/51995569527171

When people have tried to fill out the "build your own flag" order form (I even tried it), when you click submit, It doesn't allow you to submit. An error message appears briefly and says "there is an error with your order form. please correct the errors before submitting". When I tried it, all the mandatory fields have been filled out, not only that there has been at least one "flag" selection being made. I am not sure what the problem is, but I would greatly appreciate it if you could help me resolve this issue. Maybe you can see something on your end that I am not seeing. 

When I trie to do what you instructed me too in your last post, I re-did everything on wix.com and I tried to submit it from there and that same error message appeared, and I also tried submitting it directly from jot form by viewing the form, and the exact same error message came up. It is most likely due to the terms and condition widget you are using found at the bottom part of your form. It seems this widget is not functioning properly. I will submit this to our developers so they can fix it. May I suggest to replace it with a regular check box field for now found under form tools. This should fix the problem.

Sorry, I think your terms and condition widget is just outdated. This is causing the error. I would suggest to update it by removing the existing terms and condition widget on your form and replace it with a new one. This should fixed it.
