    Hi,

You can actually clone that form into your account. Just follow the link below to learn how to clone existing forms.

    http://www.jotform.com/help/42-How-to-clone-an-existing-form-from-a-URL

If you have any other questions or concerns, just let us know. Thank you for using JotForm, have a great day!

You can change the title of the form by clicking on it (in edit mode).

By the way, you can try embedding the form to your ebay store page by using the form's full source code. However, since eBay does not allow embedding of JavaScript, you must remove all scripts on the form's codes - everything in between <script and </script>

To get your form's source codes:

    1.  Go to Setup & Embed tab
    2.  Click Embed Form
    3.  Click Source

    4.  Copy the codes provided in the succeeding screen

The downside to embedding the form without scripts is that special form features will not work (validation, etc).

Please remove everything between (and including) the script tags. You may then embed the codes on the html builder.

This eBay store made use of categories to organize the items listed for sale. You can visit the following article from eBay on how this can be done.

    http://pages.ebay.com/help/sell/store-categories.html

The Part Finder displayed in the page is actually a Promotion Box where the user added a reference link to his JotForm page. Make sure though that you choose Create new promotion box  to design your own and be able to put in html codes.

    http://pages.ebay.com/help/sell/promoting-your-store.html

    Hope this information helps.
