I am looking to create a portal to our website

I am looking to create a portal to our website from a Form. Do you have any templates that allow the customer to fill out there information then click the submit button and we get their info emailed and they get redirected to a product page?

We do not want everyone accessing this, so it needs to be where they create a username and password and get redirected. Thanks

Unfortunately, log in forms cannot be created using JotForm.  Jotforms only allow for the collection of data, they cannot reference a database to authorize access based on the users credentials.  It would be possible to have them submit the form and be redirected to another web page.  But it would not be possible to collect their log in details, authorize the log in, then redirect to a page. 

Adding username or password fields will actually likely result in your account being suspended, as this is against our terms of use.  It would not be possible to create a log in portal.
