Our form is not displaying correctly

Only the banner appears when you paste the url into a window or embed it in a web page.  I've attached the URL and a screen shot of how it shows.  It has been working fine for months. Please can you take a look at this?

This is because of the condition you have, there's an error on it and a field has been deleted, the rule is not meant so the fields are not shown.

To check the existing conditions you have, navigate to "Conditions" and click the "Saved Conditions" button.

You need to edit, update or delete that field for the fields to show.
