How Can I View Original Form When I Download Submitted Forms

I would like to see the answers people give in their forms while also viewing the original questions. I can see how you can uncheck hide blank responses and check show text and headers. However, when I download the PDF those go away. I would like to make it a universal setting to always show blank answers, text, and headers.

Please advise!

When form is created inside of submission by default option Auto Hide Empty Fields is checked and Show Headers and Texts option is unchecked. So empty fields will not be shown. Also Headers and Texts won't be shown when you create PDF report. 

So if you want that empty fields are shown you need to uncheck Auto Hide Empty Fields option and if you want to Show Headers and Texts you need to check that option.

Then inside of your PDF report empty fields and Headers and Texts will be shown.

So I did this and now my PDFs will show header and text but they still do not show the empty fields. Any reason why this wouldn't work?

I have checked Submission page on your account for mentioned form and you did not uncheck Auto Hide Empty Fields option. 

To show empty fields inside of your PDF reports you need to uncheck Auto Hide Empty Fields option. 

Setting inside of your submission page should look like this.
