There are several different sliders that you can use, but the following two are the closest match:

    Sliders with calculated result

    Numeric Slider

You can see here how to add them to your form: How to Add a Widget to your Form

Do of course let us know if you happen to have any questions along the way and we would be happy to answer them.

Unfortunately, you can not pass attachments to our forum by sending an email. To add any type of image to our forum, you must go to the thread directly. You can do that by coming here: https://www.jotform.com/answers/638829

Once you do you will see what we see and will be able to add an image by following these steps (if needed): How to add screenshots/images to questions in support forum?

Once you do we will be happy to take a look at it and help you resolve the issue.
