Every time a create a form it is not accessible to view

I have created many forms but every time a try to access it I get a message stating "form not found" any help with this issue guys?

I see that you are having an issue being unable to access newly created forms. 

I found this problem occurred because our system automatically suspended your account because of the form type you are using. This was due to the fact that it almost appears to be phishing for card information which applies to Sim Cards as well since they can contain sensitive information too. But, after inspecting your form just now I don't think this was intentional.

Could you please tell us the purpose of this form? What your intention with it is? And where you plan to use it?
