Why are my images not showing up on my form (Sports Boosters Spirit Wear Order Form)? Also, I am having issues embedding in my website

I was able to see the problem when I checked your jotform http://www.jotform.us/form/52274306716151 

 

this is happening because in your products items in the Payment Form, the source URL of the item's image were incorrect.

 

You must use the direct URL path to the source images.

What you have setup was just the Google Drive shared path.. which actually wil only be shown on web viewer.

You can follow the tutorial here on how you can get the actual link to the images from your Google drive and use the direct URL link to display the images. 

Let us know if you are not able to make it work.

I have not been able to make the direct URL work.  HELP!

It seems that you are using the wrong URL to get the image form your drive. Please see the screenshot below that can help you in getting direct URL to the image.
