My form is not populating on my website

    Hello
    My form is not populating on my page correctly? -
    http://71a.1b2.myftpupload.com/contact/ 
    I have tried embedding and iframe and I must refresh the page to see it,
    how can I get around this?

    I have tested your website Inside of Google Chrome and Mozilla Firefox and your form is loading correctly. I did not need to reload the page to see the form. Also I did not had any issues with populating the form.

    Please try to clear your browser cache to see if that will resolve your issue. Also you can try using different browser.
